Flu Vaccines Oct. 23-27 in Madison City Schools: Here's The Schedule

Dear Parents/Guardians

 

Flu vaccines will be administered next week in Madison City Schools. (Schedule below) Consent forms for the Vaccinating Alabama Kids (VAKS) program are being sent home with your students’ report card. The form is also available online on the Madison City Schools Health Services webpage.

Flu vaccines are completely optional, and NO child will receive one without a completed and signed consent form.

There are no out-of-pocket costs associated with the vaccine, but your insurance will be billed. The insurance information must be complete for your child to receive the flu vaccine. If your child does not have insurance, you may indicate this on the consent form and he/she will be eligible to receive the vaccine free of charge.

Only flu SHOTs will be given.  Please make sure that your child understands this. Here is the schedule of school visits by the VAKs team. * Monday Oct. 23: West Madison Elementary. Madison Elementary, Columbia Elementary. * Tuesday Oct. 24:  Pre-K Center at Rainbow Elementary, Rainbow Elementary, Horizon Elementary.

* Wednesday Oct. 25: Mill Creek Elementary, Liberty Middle. * Thursday Oct. 26: James Clemens High School, Heritage Elementary. * Friday Oct. 27: Bob Jones High School, Discovery Middle.
